The Scandinavian Model
the process of diagnosing disease and assessing functional skills and ability, impairment-related functional limitations and disability in the elderly. This is done in relation to patient's own expectations and the demands in the environment and is the basis of treatment and rehabilitation. Multidisciplinary collaboration during the whole process is stressed as something very importaut" .
